Primary Care Networks (PCNs) are groups of GP practices working together with other local organisations, such as community, mental health, social care, pharmacy, hospital and voluntary services. They will support the needs of a population that has grown, is living longer, and may need to access local health services more often as with an ageing population, chronic disease prevalence and complex care needs will increase.

Preventative health is an important part of our overall wellbeing. We are planning webinars on ‘Preventative Health: Immunisations.’ To ensure that the webinars can answer or address current concerns, it would be useful to gather some information about your experience of immunisations.
